The last form submitted into the <br /> California Environmental Reporting System (CERS)was dated 12/28/2015 and was lacking the secondary <br /> emergency contact information. This information must be current and submitted as part of the business plan. <br /> Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, enter the <br /> correct or updated information, and submit to the EHD for approval. <br /> This is a minor violation. <br /> 5 HSC 25505(a)(1) Failed to electronically submit hazardous materials inventory for all reportable materials on site. <br /> Chemical materials (acetylene, carbon dioxide, oxygen, regular gasoline, motor oil and diesel)were found on site <br /> that have not been reported into CERS in 2016, 2017, 2018, 2019, and 2020. Any material that meets or exceeds <br /> the reportable quantity shall be reported in the facility's business plan. Immediately log into the California <br /> Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, add the materials to the hazardous material <br /> inventory, and submit to the EHD for approval. <br /> This is a minor violation. <br /> 6 HSC 25505(a)(2) Failed to electronically submit a site map with all required content. <br /> A site map was incomplete and was not submitted as part of the business plan in 2016, 2017, 2018, 2019, and <br /> 2020. The map submitted on 12/28/2020, was missing the following elements: loading area for fuels, indication of <br /> presence or absence of storm drains, access and exit points to the roundup/oil storage building, and location(s)of <br /> emergency response equipment for spill clean up. A site map shall contain a north orientation, loading areas, <br /> internal roads, adjacent streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation <br /> staging areas, hazardous material handling and storage areas, and emergency response equipment. If a site map <br /> element is not applicable for your facility then list it on the map and label as"NA". Immediately log into the California <br /> Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the correct or updated information, <br /> and submit to the EHD for approval. <br /> This is a minor violation. <br /> 9 HSC 25508(a)(1) Failed to annually review and certify that the HMBP is complete and accurate by the due date. <br /> The business plan information has not been reviewed and resubmitted in the California Environmental Reporting <br /> System (CERS) annually since the 2015 The hazardous materials inventory shall be submitted by January 15 of <br /> each calendar year and may be submitted beginning November 1 of the previous year.
