Box 241001 <br /> Lodi, CA 95241-9501 <br /> RE: Cal Waste Removal Systems Proposed Soils Recycling Facility <br /> San Joaquin County: Public Health Services, Environmental Health Division has <br /> received the submittal dated May 27, 1992 and the proposed addendum to the Report <br /> of Disposal Site Information. <br /> We will begin a detailed review of the submittal when you provide the following <br /> additional information: <br /> * A copy of the conditional use permit. <br /> # Copies of all environmental documents designed to comply with CEQA including the <br /> negative declaration,the notice of determination, and the mitigation monitoring plan. <br /> • A copy of the modified permit issued by the Air Pollution Control District. <br /> * An application for revised solid waste facilities permit. <br /> Billing will be at the rate of $53.00 per hour.
