If your exclusion is approved by the Board you shall <br /> maintain the following information at your facility for each 150 <br /> day period: <br /> ■ Quantity of waste tires maintained on the business <br /> premises at the beginning of each 150 day period. <br /> ■ Number of waste tires received during the 150 day period <br /> (this must be substantiated by receipts or other verifiable <br /> information. <br /> ■ Number of waste tires that are transported from the <br /> facility, or utilized in a manner discussed in this <br /> subsection during the 150 day period (this must be <br /> substantiated by receipts or other verifiable information) . <br /> CIWMB 500 (10/92) <br />
