Box 319 <br /> Lodi, CA 95241 <br /> Dear Mr. Vaccarezza: <br /> Year Permit Review, California Waste Removal Systems, Inc, Permit. <br /> Number 39-AA-0017 <br /> Solid waste facilities permits are required to be reviewed at least every five <br /> years by local enforcement agencies. This letter constitutes notice, pursuant <br /> to Government Code Section 66796.33(d) and Title 14, California Code of <br /> Regulations, Section 18213(a), that the permit issued to the above facility is <br /> in need of review. Our records indicate that this facility's most recent <br /> permit or permit review is dated 09/15/80. <br /> An application" for permit review, including an updated Report of Station <br /> Information or Plan of Operation, should be filed with the local enforcement <br /> agency by August 1, 1988.: Upon completion of its review, the local enforcement <br /> agency will determine if a revision of the permit is required. If a revision <br /> is necessary, you will be directed to submit an application for permit <br /> revision. If a revision is not necessary, the local enforcement agency will <br /> notify the Board and any interested person of its decision and reasons <br /> therefor. <br /> If you feel this notice is in error, please so state in a letter to the Permits <br /> Section at the letterhead address by July 1, 1988. Please include the facility <br /> name and number and the original date of permit issuance and most recent <br /> revision or review, and a brief explanation of why a review of the permit is <br /> not required at this time. <br /> If you have any questions about the permit review process, you should contact <br /> your local enforcement agency, the San Joaquin County Local Health District. <br /> Sincerely, <br /> George T.
