2 <br />-Violation 110: The uniform hazardous waste manifest (021239191JJK) provided in the response is not signed by the <br />designated facility. Can you provide a signed designated facility copy? <br />-I have faxed over said manifest to be signed by the vendor and still awaiting. <br /> <br />-Violation 113 (closed): Please note although I am closing this violation, begin keeping consolidated manifest for up to <br />three years. <br />-applicable employees have been trained on collecting waste manifest going forward. <br /> <br />-Violation 301: How was the waste generated from clean up of the used oil stains managed afterwards? i.e. were they <br />thrown away, placed in respective drums… <br />The rags used are from the Ameripride Launder service, they collect. <br /> <br />-Violation 605: The photos provided addressed all hazardous waste labels mentioned in the violation, with the <br />exception of one 55-gallon container with non-RCRA hazardous waste solids (used oil filters). I have attached a photo to <br />this email for easier reference. Please provide a statement/documentation that the hazardous waste label has been <br />corrected. All drums are labeled, all spills <br />cleaned up, all tops of drums are clean(please note the drums where in use at the time in the image) all <br />metal shavings cleaned and placed in lid container and labeled, this is placed in the maintenance shop so <br />when sanitation sweeps they know all shaving go inside that container(not pictured unfortunately I <br />thought I took the image but it did not save to my phone) <br /> <br />Please let me know what else is needed <br /> <br />
