Obsolete federal hazard categories were <br /> found in the chemical inventory section. <br /> This information must be current and submitted as part of the business plan. <br /> This violation was corrected on site. <br /> This is a minor violation. <br /> 6 HSC 25505(a)(2) Failed to electronically submit a site map with all required content. <br /> The site map was not complete as part of the business plan. The site map was fire extinguisher locations, utilities <br /> shut offs for(water, gas, power), entrances and exits, and the internal road location. <br /> A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, <br /> access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage <br /> areas, and emergency response equipment. If a site map element is not applicable for your facility then list it on the <br /> map and label as"NA". <br /> This violation was corrected on site. <br /> This is a minor violation. <br /> 7 HSC 25505(a)(3) Failed to electronically submit an adequate ER procedures for a release or threatened release. <br /> The emergency response(ER)procedures were not complete. Section C3 and E3 were not completed in the <br /> consolidated emergency response and contingency plan. C3 is the local Unified Program Agency(UPA)phone <br /> number was left blank but should be 209-468-3420. Section E3 needs the location for the evacuation staging area. <br /> A business plan shall include the following emergency response procedures for a release or threatened release of <br /> hazardous materials, including, but not limited to,the following: <br /> -immediate notification of local emergency personnel and the Environmental Health Department(EHD) <br /> -procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment <br /> -evacuation plans and procedures, including immediate notice,for the site <br /> This violation was corrected on site. <br /> This is a Class II violation. <br /> 9 HSC 25508(a)(1) Failed to annually review and certify that the HMBP is complete and accurate by the due date. <br /> The business plan information has not been reviewed and resubmitted in the California Environmental Reporting <br /> System (CERS)annually.
