Please provide the following additional information and address the following comments: <br /> 1. The Services Request needs to be signed by the billing party. Page 3 of the Application for Underground Storage Tank Installation Permit: contractor information and Fire <br /> Permit information were not provided contractor has not been awarded yet,will have to provide that info at <br /> time of permit pickup <br /> 3. Page 3 of the Application for Underground Storage Tank Installation Permit: <br /> a. Applicant signature and date were not provided updated form <br /> b. Responsible billing party information and signature were not provided updated form <br /> 4. Page 6 of the Application for Underground Storage Tank: reported capacities for premium unleaded and diesel <br /> are inconsistent with plan drawings. Provide accurate tank capacity information on all documents. <br /> 5. Section I (page 8) of the Application for Underground Storage Tank Installation Permit: all required boxes are not <br /> checked <br /> 6. Page G1.0 incorrectly states that the pipe with the longest run is diesel. This is inconsistent with the plan <br /> drawings. <br /> 7. In the engineered plans (page G5.0), UDC piping discusses removing test ports and keeping ports open for <br /> drainage. Vacuum monitored piping in a VPH system should be sealed shut at all times. <br /> a. Your response: Revised detail 4 to show vacuum tubing and ball valves that are always sealed <br /> b. Comment:The Veeder Root tubing doesn't appear to be properly connected and it has a ball valve <br /> 8. The plans indicate ball valves will be placed on vacuum tubing at multiple locations. No additional shut off <br /> valves should be installed between the monitored interstitial spaces and their respective sensors. Remove all <br /> unnecessary ball valves from the plans. <br /> 9. Provide cut sheets for the following UST system components: Flex piping model#FFUL30X18MXM. The model <br /> number is not listed on the cut sheets provided. <br /> 10. Provide current contractor's training certifications for installation and testing of all proposed UST <br /> equipment/components. <br /> 11. Provide tank charts for the tanks being used. The 8 thousand and 12 thousand tank charts provided are <br /> inconsistent with the plan drawings. <br /> 12. Provide a drawing indicating the locations of all water wells, septic systems, and storm drains. We do not have <br /> water wells on this project. We are tapping into City systems for water and sewer <br /> 13. Provide separate buoyancy calculations for each tank properly signed and stamped by a registered professional <br /> engineer. The weight used in the calculation for the split tank does not match the tank cut sheets provided. <br />
