Simon_ pate, 10/19/2020
The following work plan details activities designed to evaluate subsi, e o d '
existing underground storage tank (UST) and associated piping . The tank is a Xerxes double walled
fiberglass 6 , 000 -gallon capacity storage tank installed in 1987 . The tank was used for the dewatering
of petroleum tank bottoms from the on- site above ground storage tanks (AST ' s) . The UST does not
meet current UST spill containment regulations . There is no history of any unauthorized releases from
this tank . The tank will be triple rinsed and all contents removed . The tank will subsequently be filled
with 10 sack controlled density sand slurry and permanently closed . Prior to abandonment, soil
samples and one ground water sample will be collected and analyzed to ascertain potential
hydrocarbon concentrations prior to tank abandonment .
SCOPE OF ACTIVITIES
HEALTH AND SAFETY PLAN
A site specific Health and Safety Plan detailing known or potential hazards and emergency response
procedures will be prepared prior to field operations . All on- site personnel will review the plan and a
"tailgate " safety meeting will be conducted prior to initiation of field activities . The plan will be
maintained on-site throughout the duration of field activities . The plan will be modified if warranted
due to on-site conditions .
UNDERGROUND SERVICE ALERT NOTIFICATION
Prior to conducting any subsurface activities, the areas to be investigated will be marked and
Underground Service Alert (U . The borings
were divided into two categories depending on the assessment goal associated with each boring. Borings
designated PL- 1 , PL-2, etc . will be advanced to assess the product piping . Borings designated TP - 1 and
TP-2 will be advanced to collect soil samples and a ground water sample from the tank pit area .
During the investigation standard practices and procedures for field sampling as outlined in
Publication SW - 846 , Second Edition, United States Environmental Protection Agency will be
followed .
