S A N J D AR I N Environmental Health Department <br /> lj ;,�j i y <br /> Hazardous Materials Program Inspection Report <br /> Facility Name: Facility Address: FDat,-.Plaza liquor and Gas 2420 W TURNER RD, LODI ber 29, 2020 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 5 HSC 25505(a)(1) Failed to electronically submit hazardous materials inventory for all reportable materials on site. <br /> Approximately 5,000 gallons of diesel were found on site that have not been reported. <br /> Any material that meets or exceeds the reportable quantity shall be reported in the facility's business plan. <br /> Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, add the <br /> materials to the hazardous material inventory, and submit to the EHD for approval. <br /> This is a minor violation. <br /> 9 HSC 25508(a)(1) Failed to annually review and certify that the HMBP is complete and accurate by the due date. <br /> The business plan information has not been reviewed and resubmitted in the California Environmental Reporting <br /> System (CERS)annually.The CERS submittal for 2018, 2019, and 2020 were submitted late. <br /> Annual Hazardous Materials Business Plan review and certification is January 15* in San Joaquin County. <br /> *Pursuant to San Joaquin County Ordinance 0-13-4432. This shall be submitted by January 15 of each calendar <br /> year and may be submitted beginning November 1 of the previous year. <br /> Ensure Hazardous Materials Business Plan are submitted annually into the CERS at http://cers.calepa.ca.gov/enter <br /> the correct or updated information, and submit to the EHD for approval. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by 11/28/2020. <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br /> of having them sign. Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate <br /> ($152).
