Attached are some of the new UST forms I mentioned to you this morning, as well as links to new forms from the State <br /> Waterboard website and Unidocs website to get a copy of all forms. <br /> 1. 2020 UST DO ID Form—you only need to upload this new form to CERS if you remove the current DO identification <br /> form and upload a new DO form. Otherwise the current DO form showing Alex is fine. Anything newly uploaded after <br /> 10/1/2020 submitted into CERS for DO identification should be on this form. <br /> 2. 2020 UST Monthly DO Report—Alex should be using this form for all monthly DO reports from now on. <br /> 3. 2020 UST Statement of Understanding and Compliance - you only need to upload this new form to CERS if you <br /> remove the current form and upload a new form. Otherwise the current form in CERS is fine.
