SAN J D AR I N Environmental Health Department <br /> I Y <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> MEDLINE INDUSTRIES (MANTECA CA) 2325 W LOUISE AVE, MANTECA November 09, 2020 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 110 CCR 66262.40(a) Failed to keep signed copy of manifests from the designated facility for three years. <br /> OBSERVATION: The following designated facility signed and dated copies of Uniform Hazardous Waste Manifests <br /> (UHWM)were not found on-site. <br /> 2020: <br /> -021110823JJK(1-16-2020) <br /> -021110824JJK(1-16-2020) <br /> -021760617JJK(6-22-2020) <br /> REGULATION GUIDANCE: A hazardous waste generator shall keep a copy of each manifest signed in accordance <br /> with section 66262.23(a)for three years or until the generator receives a signed copy from the designated facility <br /> which received the waste. This signed copy shall be retained as a record for at least three years from the date the <br /> waste was accepted by the initial transporter. The hazardous waste generator must submit a copy of the manifest <br /> to the California Department of Toxic Substances Control (DTSC)within 60 days if the destination facility signed <br /> manifest copy is not received from the transporter or designated facility. <br /> CORRECTIVE ACTION: Locate the the destination facility signed manifests listed above and maintain in the facility <br /> file for three years from the date of shipping. Send a copy to the San Joaquin County Environmental Health <br /> Department(EHD). <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by 12-8-2020. <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br /> of having them sign. Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate <br /> ($152). To schedule an appointment, please call (209)468-3420. <br /> Documents provided during inspection: <br /> Waste streams found: Used oil, used oil filters, oily solids, rags contaminated with battery corrosive, universal <br /> waste batteries-alkaline <br /> Note: Facility began operating on November of 2018. No hazardous waste was produced in 2019.
