Hazelton Avenue <br />Stockton, California 95205 <br /> <br />Dear Inspectors Rivera and Vang-Lee, <br /> <br /> This letter addresses the deficiencies noted in your inspection of Sprint’s Stockton Switch, located <br />at 3807 Coronado Ave. It also addresses violations from an inspection conducted on 10/9/2020. <br /> <br />Hazardous Materials Program Inspection Report <br /> <br />Item 5, HSC 25505(a)(1) Failed to electronically submit hazardous materials inventory for all <br />reportable materials on site. <br /> <br />☒This violation was corrected ☐ This violation will be corrected by (date): <br />☐ Supporting documents included <br /> <br />Corrections were made to the site’s hazardous materials inventory. Fire suppression agent (Halon) and <br />biocide have been added to the inventory. Their locations have been added to the site maps. <br /> <br />Item 6, HSC 25505(a)(2) Failed to electronically submit a site map with all required content. <br /> <br />☒This violation was corrected ☐ This violation will be corrected by (date): <br />☐ Supporting documents included <br /> <br />New site maps have been added to the HMBP. Others have been enhanced to include internal roads, <br />access points, loading areas, and emergency shut offs. Biocide and Halon containers are now shown <br />on the maps as well. The map was submitted in CERS on 11/5/2020. <br /> <br />Item 9, HSC 25508(a)(1) Failed to annually review and certify that the HMBP is complete and <br />accurate by the due date. <br /> <br />☒This violation was corrected ☐ This violation will be corrected by (date): <br />☐ Supporting documents included <br /> <br />This violation is for a portion of the filing in 2019. The 2020 filing was made on time. There is no <br />corrective action necessary. <br /> <br />Item 11, HSC 25505(a)(4) Failed to provide or document initial and/or refresher training to <br />appropriate personnel. <br /> <br />☒This violation was corrected ☐ This violation will be corrected by (date): <br />☐ Supporting documents included <br /> <br />A paper copy of the latest HMBP has been placed at the facility. All employees at the facility <br />responsible for implementing the plan have received training. Their name and date have been recorded <br />on the training log stored at the facility. <br />
