Line C1 is missing the local Unified Program Agency <br />(UPA) phone number. Line E4 needs a box marked, locations needed for marked boxes, #8 needs to be marked in <br />section I, and section J needs a box barked. The emergency response and contingency plan has bot been <br />submitted for the years of 2019 and 2020. <br />A business plan shall include the following emergency response procedures for a release or threatened release of <br />hazardous materials, including, but not limited to, the following: <br /> - immediate notification of local emergency personnel and the Environmental Health Department (EHD) <br /> - procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br />persons, property, or the environment <br /> - evacuation plans and procedures, including immediate notice, for the site <br />Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the <br />correct or updated information, and submit to the EHD for approval. <br />This is a Class II violation. <br /> 8 HSC 25505(a)(4) Failed to establish and electronically submit a reasonable or an adequate training program. <br />A training program was not submitted for the years of 2019 and 2020. <br />Training programs for all employees and annual training, including refresher courses, shall include familiarity with <br />the facility's Emergency response plans and procedures in the event of a release or threatened release of a <br />hazardous material. Training shall cover, but not limited to, all of the following: <br />(A) Immediate notification contacts to the appropriate local emergency response personnel and to the unified <br />program agency. <br />(B) Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br />persons, property, or the environment. <br />(C) Evacuation plans and procedures, including immediate notice, for the business site. <br />These training programs may take into consideration of each employee. Immediately prepare or update the facility's <br />training program, log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, <br />upload the correct or updated information, and submit to the EHD for approval. <br />This is a Class II violation. <br /> 9 HSC 25508(a)(1) Failed to annually review and certify that the HMBP is complete and accurate by the due date. <br />The business plan information has not been reviewed and resubmitted in the California Environmental Reporting <br />System (CERS) annually. The business plan was last submitted on 2/21/2018. The business plan was not <br />submitted for 2019 and 2020. <br />The hazardous materials inventory shall be submitted by January 15 of each calendar year and may be submitted <br />beginning November 1 of the previous year.
