Environmental Health Department <br />Hazardous Materials Program Inspection Report <br />Date: <br />November 04, 2020 <br /> Facility Address: <br /> 1129 W ELEVENTH ST, TRACY <br /> Facility Name: <br /> BIG O TIRES <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br /> 4 HSC 25508(a)(1) Failed to electronically submit the Business Activities or Owner/Operator Identification page. <br />The Business Owner/Operator Identification form was not submitted for the years of 2019 and 2020. <br /> This information must be current and submitted as part of the business plan. <br />This violation was corrected on site. <br />This is a repeat violation, Class II. <br /> 5 HSC 25505(a)(1) Failed to electronically submit hazardous materials inventory for all reportable materials on site. <br />The Hazardous Materials Inventory forms for all chemicals were not complete. Obsolete federal hazard categories <br />were found in the chemical inventory section. <br />This information must be current and submitted as part of the business plan. <br />This violation was corrected on site. <br />This is a repeat violation, Class II. <br /> 6 HSC 25505(a)(2) Failed to electronically submit a site map with all required content. <br />The site map was not submitted as part of the business plan for the years of 2019 and 2020. <br /> A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, <br />access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage <br />areas, and emergency response equipment. If a site map element is not applicable for your facility then list it on the <br />map and label as “NA". Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br />This is a repeat violation, Class II. <br /> 7 HSC 25505(a)(3) Failed to electronically submit an adequate ER procedures for a release or threatened release. <br />The emergency response(ER) procedures were not submitted for the years of 2019 and 2020. The local Unified <br />Program Agency (UPA) phone number was not correct.
