Environmental Health Department <br />Hazardous Materials Program Inspection Report <br />Date: <br />November 02, 2020 <br /> Facility Address: <br /> 26342 S BANTA RD, TRACY <br /> Facility Name: <br /> MEDINA WOOD PRODUCTS INC <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br /> 5 HSC 25505(a)(1) Failed to electronically submit hazardous materials inventory for all reportable materials on site. <br />The Hazardous Materials Inventory forms for propane were not complete. The container size and max daily amount <br />were listed as listed 200 gallons. The tank capacity should be listed as 499 gallons and the the max daily amount <br />should be listed as 450 gallons. <br />This information must be current and submitted as part of the business plan. <br />This violation was corrected on site. <br />This is a minor violation. <br /> 6 HSC 25505(a)(2) Failed to electronically submit a site map with all required content. <br />The site map was not complete as part of the business plan. The site map was missing loading areas, no internal <br />roads, and no storm drains. <br />A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, <br />access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage <br />areas, and emergency response equipment. If a site map element is not applicable for your facility then list it on the <br />map and label as “NA". <br />This violation was corrected. <br />This is a minor violation. <br /> 7 HSC 25505(a)(3) Failed to electronically submit an adequate ER procedures for a release or threatened release. <br />The emergency response(ER) procedures were not complete. Line C1 was missing the local Unified Program <br />Agency UPA phone number. <br /> A business plan shall include the following emergency response procedures for a release or threatened release of <br />hazardous materials, including, but not limited to, the following: <br /> - immediate notification of local emergency personnel and the Environmental Health Department (EHD) <br /> - procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br />persons, property, or the environment <br /> - evacuation plans and procedures, including immediate notice, for the site <br />This submittal was corrected on site. <br />This is a Class II violation. <br /> 9 HSC 25508(a)(1) Failed to annually review and certify that the HMBP is complete and accurate by the due date. <br />The business plan information has not been reviewed and resubmitted in the California Environmental Reporting <br />System (CERS) annually.
