SAN JDARIN <br /> Environmental Health Department <br /> u i Y <br /> Hazardous Materials Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> ARCO 07049 800 E Kettleman Ln, Lodi November 19, 2020 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 5 HSC 25505(a)(1) Failed to electronically submit hazardous materials inventory for all reportable materials on site. <br /> A 55 gallon plastic drum of unknown liquid was found on site in the car wash chemical storage area that has not <br /> been reported. <br /> Any material that meets or exceeds the reportable quantity shall be reported in the facility's business plan. <br /> Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, add the <br /> materials to the hazardous material inventory, and submit to the EHD for approval. <br /> This is a Class II violation. <br /> 6 HSC 25505(a)(2) Failed to electronically submit a site map with all required content. <br /> A site map was not completed or submitted as part of the business plan. <br /> -2 access/exit points were indicated on the car wash chemical storage area but only one access/exit point was <br /> observed. <br /> - 1 fire extinguisher was found near the carbon dioxide storage area that was not noted on the map. <br /> -Loading areas were not indicated on the map <br /> - Internal roads were not indicated on the map <br /> -3 storm drains on the East side of the facility were not noted on the map <br /> A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, <br /> access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage <br /> areas, and emergency response equipment. If a site map element is not applicable for your facility then list it on the <br /> map and label as"NA". <br /> Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, upload the <br /> correct or updated information, and submit to the EHD for approval. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by 12/19/2020. <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br /> of having them sign. Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate <br /> ($152).
