COMPLAINANT ENDED UP GETTING REALLY ILL, <br /> THROWING UP AND HAD CHILLS AFTER AND IS STILL NOT BETTER. THE MEAT ON COMPLAINANTS BURGER <br /> HAD AN OFF TASTE SO COMPLAINANT DID NOT END UP FINISHING IT. THIS IS THE SECOND TIME <br /> COMPLAINANT GETS SICK FROM EATING AT FACILITY. THE FACILITY DID NOT HAVE PAPER TOWELS IN THE <br /> MENS RESTROOM. <br /> VIOLATIONS AND CORRECTIVE ACTIONS <br /> Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br /> 113700.All violations must be corrected within specified timeframe. Violations that are classified as"MAJOR"pose an immediate threat to public health <br /> and have the potential to cause foodborne illness.All major violations must be corrected immediate/v. Non-compliance may warrant immediate closure of <br /> the food facility. <br /> OVERALL INSPECTION NOTES AND COMMENTS <br /> FOOD ITEM --LOCATION --TEMP°F--COMMENTS <br /> No Temperature Data Collected <br /> NOTES <br /> I went to the facility and spoke to the manager who was not aware of the compliant prior to this investigation. <br /> Hamburger meat comes to the facility and is placed in the walk-in(41 F) and then in the refrigerator next to the griddle (35F). <br /> I observed the bread buns that come to the facility packaged. <br /> Per manager the tomatoes and onions are washed, cut, and cored. <br /> Pickles and cheese come to the facility packaged. <br /> I observed the soap dispenser at the hand wash sink near the front counter that is detached from wall. <br /> Provide so that soap dispenser is permanently mounted on the wall. <br /> Both hand wash sinks in the kitchen have soap and paper towels. <br /> Both Men's and women's restroom hand sinks have soap and functional air driers. <br /> The person in charge is responsible for ensuring that the above mentioned facility is in compliance with all applicable sections of the California Health and <br /> Safety Code.
