S A N J 0 A Q I I I Irl Environmental Health Department <br /> CCUNT <br /> Hazardous Materials Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> CANEPAS CAR WASH 6230 PACIFIC AVE, STOCKTON December 07, 2020 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 9 HSC 25508(a)(1) Failed to annually review and certify that the HMBP is complete and accurate by the due date. <br /> OBSERVATION: The business plan information has not been reviewed and resubmitted in the California <br /> Environmental Reporting System (CERS)annually. <br /> -the 2018 submittal was late and incomplete <br /> -the 2019 submittal was late and incomplete <br /> -the 2020 submittal was late <br /> REGULATION: The hazardous materials inventory shall be submitted by January 15 of each calendar year and <br /> may be submitted beginning November 1 of the previous year. <br /> CORRECTIVE ACTION: Immediately log into the CERS at http://cers.calepa.ca.gov/, enter the correct or updated <br /> information, and submit to the EHD for approval. <br /> This is a Class II violation. <br /> 11 HSC 25505(a)(4) Failed to provide or document initial and/or refresher training to appropriate personnel. <br /> OBSERVATIONS: The facility was unable to provide documentation of annual refresher trainings for 2018 and <br /> 2020. <br /> REGULATIONS: The business plan shall include provisions for ensuring that appropriate personnel receive initial <br /> and annual refresher training.All employees shall be trained in safety procedures in the event of a release or <br /> threatened release of a hazardous material, including, but not limited to,familiarity with the following plans and <br /> procedures: <br /> (A) Immediate notification contacts to the appropriate local emergency response personnel and to the unified <br /> program agency. <br /> (B)Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment. <br /> (C) Evacuation plans and procedures, including immediate notice,for the business site. <br /> This training shall be documented electronically or by hard copy and shall be made available for a minimum of three <br /> years. <br /> CORRECTIVE ACTION: Immediately provide employee training for appropriate personnel and submit a copy of the <br /> training records to the EHD. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by January 06, 2021 . <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br /> of having them sign. Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate <br /> ($152).
