OBSERVATION: The Hazardous Materials Inventory forms for diesel, gasoline, and carbon dioxide were not complete. • diesel contained obsolete hazard categories, did not list storage pressure or storage temperature • gasoline contained obsolete hazard categories • carbon dioxide contained obsolete hazard categories REGULATION GUIDANCE: This information must be current and submitted as part of the business plan. CORRECTIVE ACTION: This was corrected on site This is a minor violation. 6 HSC 25505(a)(2) Failed to electronically submit a site map with all required content. OBSERVATION: A site map was not completed or submitted as part of the business plan. The following items were missing from the site map: • Loading areas • Internal roads • An additional fire extinguisher was found in the stockroom • The absorbents are kept in the stockroom instead of where listed on map REGULATION GUIDANCE: A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, and emergency response equipment. If a site map element is not applicable for your facility then list it on the map and label as “NA". CORRECTIVE ACTION: This was corrected on site. This is a minor violation. 7 HSC 25505(a)(3) Failed to electronically submit an adequate ER procedures for a release or threatened release. OBSERVATION: The emergency response(ER) procedures were lacking or inadequate. • Only the first page of the Consolidated Emergency Response/Contingency Plan was uploaded into CERS, pages 2-4 were missing. REGULATION GUIDANCE: A business plan shall include the following emergency response procedures for a release or threatened release of hazardous materials, including, but not limited to, the following: - immediate notification of local emergency personnel and the Environmental Health Department (EHD) - procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to persons, property, or the environment - evacuation plans and procedures, including immediate notice, for the site CORRECTIVE ACTION: This was corrected on site.
