Hazelton Avenue SAN JOAQUIN LOCAL <br /> Stockton, CA 95201 HEALTH DISTRICT <br /> Attn: Mr. Jim Miller <br /> Gentlemen : <br /> SUBJECT: City of Lodi Well #12 Modifications <br /> As per our telephone conversation of March 17, 1981 , 1 have enclosed <br /> a permit application with a sketch of the proposed relocation of the <br /> well discharge head and control valve piping. This includes an <br /> extension of the 1/4" wall steel conductor casing, a 5" thick annular <br /> concrete seal to the ground surface, and construction of a new dis- <br /> charge pedestal (minimum 1 ' in height AGL) .
