Name of 24-hour contact person and phone number. <br /> C. Location of disposal area. <br /> D. Name of property owner, address and phone number. <br /> E. Date of initial haul. <br /> F . Type of operation. (e.g. land application or livestock feed) <br /> G. Specific type of waste to be transported. <br /> H. Quantity of waste to be transported. (cu. yards) <br /> I. Provide the source of all waste materials including the name, <br /> address , and phone number of all responsible contact persons <br /> at the supplier level. <br /> J. List the hours and days per week this disposal site will be <br /> in operation. <br /> K. Describe the soil preparation procedures and land application <br /> (or other) technique you plan to institute at the proposed <br /> agricultural disposal site. <br /> L. Describe the type of pesticide and the method of applying this <br /> pesticide at the proposed agriculture disposal site should <br /> conditions warrant this action. <br /> M. Describe all contingency plans for selecting alternative <br /> sites and provide the location of all alternative sites <br /> should weather conditions warrant this action. <br /> N. Describe plans for accomodating weather conditions which may <br /> be adverse to your proposed waste handling procedure. <br /> Provide the following information on a scaled drawing not less than <br /> 1" = 600' (Parcel Maps that meet this requirement are available at <br /> the San Joaquin County Assessor' s Office) : <br /> A. Identify the agriculture disposal site location and the number <br /> of acres . <br /> B. Identify all wells , ponds , lakes , reservoirs , streams , drainage <br /> courses or other waterways within 200 ' of the proposed agriculture <br /> disposal site. <br />
