OBSERVATION: The designated operator employee training for facility employees has not performed. REGULATION GUIDANCE: The designated operator shall train facility employees in the proper operation and maintenance of the UST system at least once every 12 months. The training shall be conducted before the individual performs the duties of a facility employees.The training shall include, but is not limited to: 1. Operation of the UST system in a manner consistent with the facility's best management practices 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan 3. Employee's role with regard to spills and overfills as specified in the facility's response plan 4. Name of the contact person(s)for emergencies and monitoring equipment alarms Initial training shall also include a site-specific practical demonstration that provides both sharing information and evaluating knowledge for responding to emergencies, leak detection, monitoring alarms, spills and overfills. At least one of the facility employees present during operating hours shall have current training. CORRECTIVE ACTION: Ensure that employees have been trained by the designated operator, maintain the list on site, and submit a copy of the training records to the EHD. This is a Class II violation. 308 CCR 2632(c), 2634(d), 2636(f) Failure of the leak detection equipment to have an audible and visual alarm. The 87 float and chain in the 3/4 UDC failed to stop the flow of product at the dispenser when tested. REGULATION GUIDANCE:All monitoring equipment shall be maintained to activate an audible and visual alarm or stop the flow of product at the dispenser when it detects a leak. CORRECTIVE ACTION: The service technician adjusted the float and chain and verified functionality. This is a Class II violation. 309 CCR 2666(b)(2), 2636(f)(1) Failure to have continuous monitoring that activates audible-visual alarm or stops flow at dispenser. OBSERVATIONS: The UDCs failed to have monitoring equipment that activates an audible and visual alarm or stop the flow of product at the dispenser when it detects a leak. On 07/21/2021, per service technician David Winker,the monitoring system certification testing could not be performed because the UDCs did not have sensors and the floats did not have chains connected to the Schrader valve. REGULATION GUIDANCE:All monitoring equipment shall be maintained to activate an audible and visual alarm or stop the flow of product at the dispenser when it detects a leak. CORRECTIVE ACTION: The floats and chains were tested and passed during the inspection on 07/28/2021. This is a Class II violation. Overall Inspection Comments: An inspection checklist was provided to the facility operator at the time of inspection.
