1 11 . 3 Respondent corrects any violation (s ) cited on the Site by the CUPA <br /> 2 immediately and provides documentation of correction to the CUPA <br /> 3 within 30 days of citation . <br /> 4 12 . Respondent' s check (s ) shall be made payable to " San Joaquin County <br /> 5 Environmental Health Department , " and shall be delivered together with the <br /> 6 attached Payment Voucher to ; <br /> 7 San Joaquin County Environmental Health Department <br /> 8 Certified Unified Program CUPA <br /> 9 1868 E . Hazelton Avenue <br /> 10 Stockton , California 95205 <br /> 11 13 . Respondent shall send a photocopy of the check to the following CUPA <br /> 12 personnel assigned to this matter, Michelle Henry, San Joaquin County <br /> 13 Environmental Health Department <br /> 14 14 . If Respondents fail to make payment( s ) as provided above , Respondents <br /> 15 agrees to pay interest at the rate established pursuant to HSC § 25360 . 1 <br /> 16 and to pay all costs incurred by the CUPA in pursuing collection including <br /> 17 attorney' s fees . <br /> 18 15 . All additional CUPA costs will be billed at the Board 's approved hourly rate . <br /> 19 OTHER PROVISIONS <br /> 20 16 . Additional Enforcement Actions : By agreeing to this Consent Order, the <br /> 21 CUPA does not waive the right to take further enforcement actions , except <br /> 22 to the extent provided in this Consent Order. <br /> 23 17 . Penalties for Noncompliance : Failure to comply with the terms of this <br /> 24 Consent Order may subject Respondents to civil penalties and/or punitive <br /> 25 damages for any costs incurred by the CUPA or other government agencies <br /> 26 as a result of such failure , as provided by HSG § 25404. 1 . 1 and other <br /> 27 applicable provisions of law . <br /> 28 18 . Parties Bound : This Consent Order shall apply to and be binding upon <br /> - 4 - <br /> CONSENT ORDER Associated Tractor Service Inc . <br />
