SAN JOAQUIN <br />CCUNT <br />Environmental Health Department <br />AMENDED <br />4/14/2021 <br />Small Quantity Hazardous Waste Generator Inspection Report <br />Facility Name: <br />Facility Address: <br />Date: <br />ASSOCIATED TRACTOR SERVICE INC <br />34 N VENTURA AVE, STOCKTON <br />April 14, 2021 <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />Item # <br />Remarks <br />111 <br />CCR 66262.42(a)(c)(d) Failed to comply with uniform hazardous waste manifest exception requirements. <br />OBSERVATION: The facility did not have exception reports on site for Uniform Hazardous Waste Manifest <br />(UHWM) 006453758GBF (4/7/2021). <br />REGULATION GUIDANCE: <br />(a) A generator who does not receive a copy of the manifest with the handwritten signature of the owner or operator <br />of the designated facility within 35 days of the date the waste was accepted by the initial transporter shall contact the <br />transporter and/or the owner or operator of the designated facility to determine the status of the hazardous waste. <br />(c) A hazardous waste generator who does not receive a copy of the manifest with the handwritten signature of the <br />owner or operator of the facility to which the generator's waste is submitted, within 60 days from the date that the <br />hazardous waste was accepted by the initial transporter, shall submit to the department a legible copy of the <br />manifest, with some indication that the generator has not received confirmation of delivery. <br />(d) Generators shall submit the information to the department at: DTSC Report Repository, Generator Information <br />Services Section, P.O. Box 806, Sacramento, CA 95812-0806 <br />CORRECTIVE ACTION: Locate a copy of the destination facility signed manifest and send a copy to the San <br />Joaquin County Environmental Health Department (EHD). If the destination facility signed manifests cannot be <br />found then send to DTSC a copy of each generator signed manifest and a statement that the designated facility <br />signed copy was not received by the generator. DTSC address is DTSC Report Repository, Generator Information <br />Services Section, PO Box 806, Sacramento, CA 95812-0806. Provide a corrective action statement to the EHD <br />within 30 days. <br />This is a Class II violation. <br />301 <br />CCR 66262.34(d)(2) Facility not maintained to minimize the release of a hazardous waste. <br />OBSERVATION: Oil saturated dirt was observed in several spots throughout the yard. <br />REGULATION GUIDANCE: A hazardous waste generator must maintain and operate its facility to minimize the <br />possibility of a fire, explosion, or any unplanned sudden or non -sudden release of hazardous waste or hazardous <br />waste constituents to air, soil, or surface water which could threaten human health or the environment. <br />CORRECTIVE ACTION: Immediately fix the leaking equipment and clean up the soil. Manage the waste according <br />to Title 22 California Code of Regulations for hazardous waste. Submit a statement and supporting documentation <br />to the San Joaquin County Environmental Health Department describing the storage and final disposition of the <br />cleaned up waste. <br />This is a repeat violation, Class II. <br />Overall Inspection Comments: <br />An inspection report was provided to the facility operator on April 14, 2021. Later the same day, the report was <br />amended by the EHD.
