QUEST INDUSTRIES LLC,2518 E BOEING WAY, CERS ID: 10184627, PR0538472
Corrective Action Statements
RE: November 04,2020 Hazardous Waste inspection report
The facility is phasing out its use of 55-gallon drums for hazardous material storage and replacing them
with plastic totes and lined cardboard containers provided by Clean Harbors(photos attached).Liquid paint
waste totes and solid paint waste cardboard boxes will be closed when not adding or removing waste.
Containers which have been altered have been cut up and disposed of in the solid paint waste containers.
Existing containers have been provided with adequate lids before drums are full (i.e. gasket and locking
drum funnels on closed-head containers, latching drum lids on open-head containers)and after drums have
been filled(nut and bolt)to provide secure closure.
Violation 501-Failed to completely label stationary hazardous waste tanks
❑ Corrected
❑ Violation Corrected by:
0 Supporting Documents Included(analytical results)
Corrective Action:Wash water waste was sampled by Clean Harbors on June 23,2020.Attached analytical
results supported a non-hazardous waste determination. Quest asserts that non-hazardous wastewater was
treated onsite. As a precaution,Quest halted operation of the evaporator as of November 4,2020,to further
investigate the process. If characterization at the point of waste generation results in a hazardous waste
determination, Quest will obtain permit coverage prior to starting operation of the evaporator unit and will
include the required labels.
Violation 502-Failed to comply with new hazardous waste tank system assessment requirements
❑ Corrected
❑ Violation Corrected by:
0 Supporting Documents Included(analytical results)
Corrective Action:Wash water waste was sampled by Clean Harbors on June 23,2020.Attached analytical
results supported a non-hazardous waste determination.
