The generator shall within 30 days of each shipment of hazardous waste submit to DTSC a <br /> legible copy of each manifest used.The mailing address is: DTSC Generator Manifests, Department of Toxic <br /> Substances Control, P.O. Box 400, Sacramento, CA 95812-0400. <br /> CORRECTIVE ACTION: Immediately mail a copy of the listed UHWMs and ensure that copies of all generator <br /> signed UHWMs are sent to DTSC within 30 days of shipping the hazardous waste. Also provide a corrective action <br /> statement and supporting documentation to the San Joaquin County Environmental Health Department(EHD). <br /> This is a minor violation. <br /> 115 CCR 66262.40(a) Failed to keep signed copy of manifests from the designated facility for three years. <br /> OBSERVATION: <br /> 1. Designated facility signed and dated copies of Uniform Hazardous Waste Manifests(UHWM)were not found <br /> on-site. The missing manifests are: <br /> -014649293FLE (4/12/2021) <br /> 2. Chad Yelm, Production Manager, stated the facility had a clean out of the processing line baths 2,5 and 8 within <br /> the past two months. The manifest for the waste disposal was not available for review. <br /> REGULATION GUIDANCE: A hazardous waste generator shall keep a copy of each UHWM signed in accordance <br /> with section 66262.23(a)for three years or until the generator receives a signed copy from the designated facility <br /> which received the waste. This signed copy shall be retained as a record for at least three years from the date the <br /> waste was accepted by the initial transporter. The hazardous waste generator must submit an exception report to <br /> the California Department of Toxic Substances Control (DTSC)within 45 days if the destination facility signed <br /> manifest copy is not received from the transporter or designated facility. <br /> CORRECTIVE ACTION: Locate the destination facility signed manifests listed above and maintain in the facility file <br /> for three years from the date of shipping.
