The following items need to be <br /> addressed/corrected: <br /> -The Tank Overfill Prevention is checked off as"audible/visual alarms"and"fill tube shut-off valve"but the additional <br /> comment section lists that ball floats are used ("Overfill prevention: External A/V alarm set at 90%tank capacity and <br /> ball floats").This additional comment/information does not match what is checked off. (Ball floats were removed <br /> during 11/12/2021 overfill prevention testing). <br /> -For all three tanks Under Dispenser Containment(UDC)Monitoring lists the following incorrect information: <br /> *UDC Panel Manufacturer:Veeder-Root(floats and chains do not have a panel) <br /> *UDC Panel Model:#TLS 350 (floats and chains do not have a panel) <br /> *UDC Leak Sensor Model: Bravo(should be floats and chains) <br /> REGULATION GUIDANCE: The monitoring plan must be uploaded to the California Environmental Reporting <br /> System (CERS). <br /> CORRECTIVE ACTION: Immediately log into CERS, make the necessary changes, and submit for review by the <br /> EHD. <br /> This is a repeat violation, Class II. <br /> 107 HSC 25292.2; CCR 2711(a)(11)Failure to submit and maintain complete and current financial responsibility <br /> documents. <br /> OBSERVATION: Financial responsibility documents have not been submitted to the EHD.
