Pretreatment Industrial Users <br />General Permit Conditions Page <br />10of14 <br />a. User notification in case of upset. The Industrial User shall submit the following <br />information to the City of Stockton, Environmental Control Office within 24 hours <br />of becoming aware of the Upset (if this information is provided orally, a written <br />submission must be provided within five days): <br />A description of the indirect discharge and cause of noncompliance; <br />2. The period of noncompliance, including exact dates and times or, if not <br />corrected, the anticipated time the noncompliance is expected to continue; <br />3. Steps being taken and/or planned to reduce, eliminate and prevent <br />recurrence of the noncompliance. <br />b. User responsibility in case of upset. The Industrial User shall control production or all <br />discharges to the extent necessary to maintain compliance with local andior <br />categorical Pretreatment Standards upon reduction, loss, or failure of its treatment <br />facility until the facility is restored or an alternative method of treatment is provided. <br />This requirement applies in the situation where, among other things, the primary <br />source of power of the treatment facility is reduced, lost or fails. <br />35. Signatory Requirements: All applications, reports, follow-up reports, supplemental reports, <br />documentation and other information submitted to the City of Stockton shall contain the <br />following certification statement signed by an authorized or duly authorized representative <br />of the user (original signature required) as specified in SMC 13.08.080 and 13,08.320: <br />°1 certify under penalty of law that this document and all attachments were <br />prepared under my direction or supervision in accordance with a system <br />designed to assure that qualified personnel properly gather and evaluate <br />the information submitted. Based on my inquiry of the persons directly <br />responsible for gathering the information, the information submitted is, to <br />the best of my knowledge and belief, true, and accurate, and complete. I <br />am aware that there are significant penalties for submitting false <br />information, including the possibility of fine and imprisonment for knowing <br />violations. " <br />a. The certification statement shall be signed by an authorized or duly authorized <br />representative of the user as follows: <br />If the user is a corporation: <br />A. The president, secretary, treasurer, or a vice president of the <br />corporation in charge of a principal business function, or any other <br />person who performs similar policy or decision making functions for <br />the corporation; or <br />B. The manager of one or more manufacturing, production, or <br />operating facilities, provided the manager is authorized to make <br />
