Page 2of3 <br /> REPORT <br /> The report shall include a summary of the inspection findings, applicable inspection checklists per API <br /> 653 / STI guidance, CAD drawings illustrating the shell layout with location of significant findings, NDT <br /> data with applicable data tables including location and remaining thickness of any recordable internal <br /> metal loss and photographs. This tank is considered to be a "New Tank System". Certification in <br /> accordance with Title 22 66265.192(a) and (h) will be provided as part of the report. This <br /> certification will be signed and stamped by a Registered Civil Engineer in California. NOTE: A <br /> PDF copy of the final report are to be submitted to Kraft Heinz Company within fifteen (15) business <br /> days of the last day on-site for their review. Once approved, then a hard copy will be submitted. <br /> PERSONNEL <br /> The crew shall consist of an Inspector certified per API Standard 653 / STI Adjunct / NDT technician <br /> (UT Level II). <br /> TANK PREPARATION <br /> 1) The surfaces or welds to be inspected by visual or ultrasonic examinations should be free from <br /> scale, rust, dirt, oil, grease, or other extraneous materials, which may interfere or limit them. <br /> 2) General assessment of safe access around this tank will be made by the crew supervisor prior to <br /> performing any work thereon. <br /> 3) Appropriate safety evaluations and lock and tag outs completed, including confined space <br /> procedures in accordance with 29 CFR 1910.146 and OSHA. <br /> COST OF SERVICES <br /> We are pleased to conduct this verification and engineering evaluation for compliance with DTSC <br /> regulations for the following lump sum price: <br /> Tank ID Product Capacity Tank Year Built Price <br /> (Gal.) Tank preparation; or <br /> B. Safety or Site Specific training lasting longer than '/2 hour/ permitting /security delays <br /> C. Delays due to availability or access to restricted areas. <br />
