OBSERVATION: The designated operator inspections for the following months were performed more than 30 days after the previous inspection: - Initial inspection on 05/16/2020, next inspection performed on 06/18/2020 (33 days) - Initial inspection on 07/15/2020, next inspection performed on 08/19/2020 (35 days) - Initial inspection on 09/16/2020, next inspection performed on 10/17/2020 (31 days) REGULATION GUIDANCE: The designated operator visual inspections shall be conducted within 30 days or less of the previous inspection. CORRECTIVE ACTION: Ensure that all designated operator inspections are performed within the required time frames. -------------------------------------- -------------------------------------- OBSERVATION: Designated operator reports were not signed within the required time frame. The 04/18/2020 and 08/19/2020 reports were not signed by the owner/operator. REGULATORY GUIDANCE:Within 48 hours of the completion of the designated UST operator visual inspection, the designated operator shall sign and provide the owner or operator with a copy of the inspection report. Within 72 hours of being provided a signed copy of the report,the owner or operator shall provide a description of each corrective action taken or to be taken and shall sign and date the report, acknowledging the identified compliance issues. CORRECTIVE ACTION: Ensure all designated operator inspection reports are completed and signed within the required time frame in the future. This is a repeat violation, Class ll. 209 CCR 2715(c)(3), 2715(c)(4) Failure to maintain a copy of the"Facility Employee Training Certificate"on-site. OBSERVATION: The Facility Employee Training Certificates had the following issues: -02/19/2020:the training date(7/7/20)for Rachel Makowsky was dated after the 02/19/2020 date of training -08/19/2020:the training dates for Timothy Taylor(9/15/2020), Tracy Shaw(9/26/2020), Simone Binoir(11/18/2020), and Sara Belasco(11/18/2020)were all dated after the 8/19/2020 date of training -Sara Belasco's initial training date was on 11/18/2020, after her date of assuming responsibility(3/10/2020) -Simone Binoir's initial training was on 11/18/2020. more than 30 days after her date of assuming responsibility on 12/07/2017 REGULATION GUIDANCE:All site employees must be trained prior to taking responsibility as a facility employee if after October 1, 2018, or within 30 days of hire if prior to October 1, 2018. The Facility Employee Training Certificate shall contain accurate information for all employees trained. CORRECTIVE ACTION: Update the Facility Employee Training Certificate to contain the accurate information and submit a copy of the updated form to the EHD. This is a minor violation.
