SAN JOARU <br />.�_,u;� I <br />Environmental Health Department <br />Underground Storage Tank Program Inspection Report <br />Facility Name: <br />Facility Address: —Fe: <br />EI Dorado Food Mart <br />1901 S EL DORADO ST, STOCKTON <br />ptember 30, 2021 <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />Item # <br />Remarks <br />REGULATION GUIDANCE: During the inspection, the designated operator shall check that all required testing and <br />maintenance for the UST system have been completed and document the dates they were done. <br />CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of <br />the required information on the reports. <br />This is a repeat violation, Class II. <br />209 <br />CCR 2715(c)(3), 2715(c)(4) Failure to maintain a copy of the "Facility Employee Training Certificate" on-site. <br />OBSERVATION: The Facility Employee Training Certificate dated 2/11/2021 states Brenda Campa was trained on <br />5/1/2021. Brend Campa did not start working at the facility until 5/1/2021. <br />The Facility Employee Training Certificate dated 3/25/2021 states Brenda Zenger was trained on 8/25/2021. Brend <br />Zenger did not start working at the facility until 8/25/2021. <br />REGULATION GUIDANCE: All site employees must be trained prior to taking responsibility as a facility employee if <br />after October 1, 2018, or within 30 days of hire if prior to October 1, 2018. The Facility Employee Training <br />Certificate shall contain accurate information for all employees trained. <br />CORRECTIVE ACTION: Update the Facility Employee Training Certificate to contain the accurate information and <br />submit a copy of the updated form to the EHD. <br />This is a Class II violation. <br />210 <br />CCR 2715(c) Failure to provide initial training, train every 12 months, and have a trained employee present. <br />OBSERVATION: The designated operator employee training for Brenda Zenger was not performed. <br />REGULATION GUIDANCE: The designated operator shall train facility employees in the proper operation and <br />maintenance of the UST system at least once every 12 months. The training shall be conducted before the <br />individual performs the duties of a facility employees. The training shall include, but is not limited to: <br />1. Operation of the UST system in a manner consistent with the facility's best management practices <br />2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br />3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br />4. Name of the contact person(s) for emergencies and monitoring equipment alarms <br />Initial training shall also include a site-specific practical demonstration that provides both sharing information and <br />evaluating knowledge for responding to emergencies, leak detection, monitoring alarms, spills and overfills.
