OBSERVATION:A copy of the UST operating permit was not found on site. REGULATION GUIDANCE:A copy of the UST operating permit and all attachments and conditions shall be retained at the facility. CORRECTIVE ACTION: This violation was corrected on site. The EHD provided a copy of the operating permit at the time of the inspection. It was printed and made available on site. No further action needed. This is a Class II violation. 208 CCR 2716(a-e) Failure to comply with the designated operator inspection requirements. OBSERVATION: The owner/operator failed to document follow-up action to the compliance issues listed for the following reports: 03/03/2020, 03/31/2020, 04/29/2020, 07/20/2020, 08/17/2020, 09/14/2020, 12/08/2020, and 01/05/2021. REGULATORY GUIDANCE:Within 48 hours of the completion of the designated UST operator visual inspection, the designated operator shall sign and provide the owner or operator with a copy of the inspection report. Within 72 hours of being provided a signed copy of the report,the owner or operator shall provide a description of each corrective action taken or to be taken and shall sign and date the report, acknowledging the identified compliance issues. CORRECTIVE ACTION: Ensure all designated operator inspection reports are completed and signed within the required time frame in the future. ----------------------- ----------------------- OBSERVATION: The designated operator failed to check and note the correct test dates for the monitoring system certification on the 08/17/2020, 09/14/2020, 10/12/2020, 11/10/2020, 12/08/2020, and 01/05/2021 designated operator inspection reports. The monitoring system certification test date was documented as 01/08/2020, however,the monitoring system certification was conducted on 07/20/2020. REGULATION GUIDANCE: During the inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of the required information on the reports. This is a repeat violation, Class II.
