S A N J 0 A Q I I I Irl Environmental Health Department <br /> CCUNT <br /> Underground Storage Tank Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> fast and easy mart#103 T8660 LOWER SACRAMENTO RD, STOCKTON June 04, 2021 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 206 CCR 2712(i), 2634(e)Failure to have a current UST Response Plan available on site. <br /> OBSERVATION:An approved copy of the response plan was not available for inspection. <br /> REGULATION GUIDANCE:A copy of this plan shall be accessible on site at all times. <br /> CORRECTIVE ACTION: Immediately locate and retain a copy on site. <br /> This is a repeat violation, Class II. <br /> 208 CCR 2716(a-e) Failure to comply with the designated operator inspection requirements. <br /> OBSERVATION: The designated operator failed to check and note all required items on the visual inspection <br /> reports.The incorrect and missing information included the following from the inspection reports:the owner/operator <br /> failed to provide follow-up action for the compliance issue identified on the 04/09/2021 report <br /> REGULATION GUIDANCE: During the visual inspection,the designated operator shall check that all required <br /> testing and maintenance for the UST system have been completed and shall check all required items on the report. <br /> CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including <br /> all of the required information on the reports. <br /> This is a repeat violation, Class II. <br /> 319 CCR 2635, 2637.2,2665, 2712 Failure to comply with overfill prevention equipment requirements. <br /> OBSERVATION: The fill tube shut-off valve used as the overfill prevention system is not currently functional on the <br /> 91 (premium)UST. <br /> REGULATION GUIDANCE:All UST systems must be equipped with an approved,functional overfill prevention <br /> device at all times during delivery operations. <br /> CORRECTIVE ACTION: Immediately discontinue deposition of petroleum into this tank until the component is <br /> repaired or replaced by a properly licensed, trained, and certified contractor under permit and inspection of the EHD. <br /> If the failed component can't be replaced immediately,there is a possibility that the 91 UST system may be red <br /> tagged to prevent fuel inputs. <br /> This is a repeat violation, Class II. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by July 4, 2021 . <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br /> of having them sign. Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate <br /> ($152).
