Testing has not been completed and is 2 months past <br /> due. <br /> REGULATION GUIDANCE: These tests are required once every 12 months. <br /> CORRECTIVE ACTION: Immediately schedule these tests and provide 48 hours notification to the EHD. Please be <br /> aware that all EHD staff time associated with a follow up inspection will be billed at our current hourly rate($152). <br /> This is a Class II violation. <br /> 113 CCR 2637(a-d), 2712(b)(1)(F) Failure to conduct secondary containment testing. <br /> OBSERVATION: Secondary containment testing was last performed on 3/28/2018 and was due by March 2021. <br /> Testing has not been completed and is 3 months past due. <br /> REGULATION GUIDANCE: Secondary containment testing is required once every 36 months. <br /> CORRECTIVE ACTION: Immediately schedule this test and provide 48 hours notification to the EHD. <br /> This is a Class II violation. <br /> 118 CCR 2637.2(d&e), 2665(a&b) Failure to submit the"Overfill Prevention Equipment Inspection Report Form"within <br /> 30 days. <br /> OBSERVATION: Overfill Prevention Equipment testing was performed on 2/15/2021 and a test report was <br /> submitted on 4/27/2021, 2 months late. <br /> REGULATION GUIDANCE:A copy of the test report must be submitted within 30 days of the tests. <br /> CORRECTIVE ACTION: Take all necessary precautions to ensure testing and submittal of test reports are <br /> performed in a timely manner. <br /> This is a minor violation. <br /> FA0003290 PR0231801 SCO01 06/23/2021 <br /> EHD 23-01 Rev.9/16/2020 Page 5 of 7 Underground Storage Tank Program OIR <br /> 1868 E.
