Training of the procedures for the mitigation <br /> of a release or threatened release was last completed on 2/24/20. <br /> REGULATION GUIDANCE: The business plan shall include provisions for ensuring that appropriate personnel <br /> receive initial and annual refresher training. All employees shall be trained in safety procedures in the event of a <br /> release or threatened release of a hazardous material, including, but not limited to, familiarity with the following <br /> plans and procedures: <br /> (A) Immediate notification contacts to the appropriate local emergency response personnel and to the unified <br /> program agency. <br /> (B) Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment. <br /> (C) Evacuation plans and procedures, including immediate notice, for the business site. <br /> This training shall be documented electronically or by hard copy and shall be made available for a minimum of three <br /> years. <br /> CORRECTIVE ACTION: Immediately provide employee training for appropriate personnel and submit a copy of the <br /> training records to the EHD. <br /> This is a repeat violation, Class II. <br /> 12 HSC 25505(c) Failed to have the HMBP readily available to facility or EHD personnel. <br /> OBSERVATION: Facility could not provide the inventory information upon request. Employees do not have access <br /> to the inventory information on-site. <br /> REGULATION GUIDANCE: The emergency response plans and procedures, the inventory of information required <br /> by this article, and the site map required by this section shall be readily available to personnel of the business or the <br /> unified program facility with responsibilities for emergency response or training pursuant to this section. <br /> CORRECTIVE ACTION: Immediately make arrangements to have the business plan readily available for review. <br /> This is a repeat violation, Class ll. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by May 14, 2021. <br /> Add the cylinder of unknown material, if applicable, found outside at the north east corner of shop 1 to the <br /> inventory and site map. <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br /> of having them sign. <br /> FA0009152 PR0519423 SCO01 04/14/2021 <br /> EHD 19-01 Rev.9/16/2020 Page 3 of 4 Hazardous Materials Program OIR <br /> 1868 E.
