E <br />-Final sample will be placed in a shipping container, labeled and all appropriate testing <br />forms filled out. <br />Hours of operation <br />The hours of operation for this facility are: Monday - Saturday, 7 a.m. — 4 p.m. The facility is <br />closed on Sundays and the following holidays: New Years Day, Presidents Day, Memorial Day, <br />independence Day, Labor Day Thanksgiving and Christmas. The facility may operate outside <br />normal business hours occasionally if there is an emergency, special occurrence or to <br />accommodate community clean ups. If an event is to take place, the LEA will be notified at <br />least 48 prior to, or 48 hours after, the event. The activity will be logged as a special <br />occurrence. <br />Facility Records <br />All incoming and outgoing material is weighed, logged and document as to origin, type or <br />destination. Certified scale taps are sequential and required to be kept for five years. Special <br />occurrences are logged in a separate ledger. The date, time and circumstances described to <br />detail files, injury or any unusual issues and kept in this ledger as well. Special occurrences are <br />reported to and logged by the supervisor on duty, including any special occurrences <br />encountered during operation and methods used to resolve problems arising from these <br />events, including details of all incidents that required implementing emergency procedures. <br />The logs are kept in the scale house and are available for inspection at the scale house by <br />authorized representatives of CalRecycle, the LEA, local health or other agencies during normal <br />working hours. <br />Any public complaints are also logged and include: the nature of the complaint, date received, <br />if available the name, address and phone number of the person making the complaint, and any <br />actions taken to resolve the complaint. <br />The facility logs also include the quantity and type of feedstock received and quantity of <br />compost and chipped and ground material produced. The number of load checks performed <br />and loads rejected are also included in this document. <br />Records also include all test results generated by compliance with Article 7 of this Chapter, <br />including but not limited to, metal concentrations, fecal coliform and Salmonella sp. densities, <br />temperature measurements, and dates of windrow turnings and pathogen reduction methods. <br />These records include any serious injury to the public occurring on-site and any complaint of <br />adverse health effects to the public attributed to operations. Serious injury means any injury <br />that requires inpatient hospitalization for a period in excess of 24 hours or in which a member <br />of the public suffers a loss of any member of the body or suffers any degree of permanent <br />disfigurement. Training and instruction of personnel are also included and detailed in these <br />records. <br />H. <br />
