The incorrect and missing information included the following from the inspection reports: <br /> -The DO (designated operator) reports of 6/01 /20, 06/29/2020 , 09/01 /2020 , and 02/08/2021 the OWNER / <br /> OPERATOR failed to provide a name , signature, and date within required time frame of receiving the DO report <br /> under the ACKNOWLEDGMENT OF COMPLIANCE ISSUES section <br /> - The DO report of 10/26/2020 did not include the owner/operator name . <br /> REGULATION GUIDANCE : During the visual inspection , the designated operator shall check that all required <br /> testing and maintenance for the UST system have been completed and shall check all required items on the report . <br /> CORRECTIVE ACTION : Ensure that designated operators performing visual inspections at this facility are including <br /> all of the required information on the reports . <br /> 2 ) OBSERVATION : The designated operator inspections for the following months were performed more than 30 <br /> days after the previous inspection : <br /> - Initial inspection on 7/23/2020 , next inspection performed on 09/01 /2020 (70 days) between the two dates . <br /> REGULATION GUIDANCE : The designated operator visual inspections shall be conducted within 30 days or less of <br /> the previous inspection . <br /> CORRECTIVE ACTION : Ensure that all designated operator inspections are performed within the required time <br /> frames . <br /> This is a repeat violation , Class Il . <br /> Overall Inspection Comments : <br /> An inspection report was provided to the facility operator on the day of inspection . The report was amended <br /> and replaces the initial report . <br /> * Violation # 122 removed . <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation , and any supporting <br /> paperwork , within 30 days of receiving the amended inspection report . <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br /> of having them sign . Starting September 1 , 2018 , all in-office CERS help will be provided at EHD hourly rate <br /> ($ 152) .
