SAN J O A Q I I I N Environmental Health Department <br /> L -0 U N T Y I�I AMENDED <br /> 10/28/2019 <br /> RCRA Large Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> Dollar Tree Distribution Center-Stockton 1 1122 Runway Dr, Stockton October 14, 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> Wash, one of which was open and had signs of past leakage on its cap(IMG 8856-7).A metal beam next to the <br /> box of shampoo had green staining that appeared to be the V05 shampoo(IMG 8858). <br /> 31) Found on the MOD floor grate, a damaged leaky bottle of Kar&Danver Body Wash, missing its pump <br /> dispenser(IMG 8861-2). <br /> 32)On the north side of the facility, several unlabeled closed cardboard boxes were observed stacked two rows <br /> high on a pallet beneath several silver 16 fl oz bottles of SMB Synthetic Blend 1OW-30 SAE Motor Oil (IMG 8872, <br /> 8878).At least seven of the cardboard boxes were seen with dark staining/leakage(IMG 8872). <br /> 33)On the north side of the facility, several unlabeled closed cardboard boxes were observed stacked four rows <br /> high on a pallet(IMG 8873).At least 15 of the cardboard boxes were seen with dark staining/leakage(IMG 8873-4, <br /> 8879-81).The boxes contained silver16 fl oz bottles of SMB Synthetic Blend 1OW-30 SAE Motor Oil (IMG 8875-7). <br /> 34)A damaged open container of Comet cleaning powder containing bleach, and spillage of the Comet powder, <br /> was found on the floor of the facility(IMG 8929-30). <br /> 35)Spillage of an unknown dark brown gel material was observed on the facility floor near Pick Module 1 Zone <br /> A-B, where Charell Gaines,an employee, was observed sorting through damaged and un-damaged product(IMG <br /> 8936-7, 8940-1, 8943-44). Sherry Aebersold, Environmental Health &Safety Compliance Manager,was told by an <br /> employee that the spillage was grease that was leaking from a mechanical part on the conveyor belts at Pick <br /> Module 1. However, when Rebecca Corona, Hazmat employee,was asked by Sherry Aebersold and Gustavo <br /> Garcia, Operations Manager,to dump a dumpster of trash into the trash compactor,two leaky 8 oz containers of <br /> the dark brown gel material and labeled Softee Extra Hold Protein Styling Gel,were found in the dumped trash in <br /> the compactor(IMG 8964, 8967-9).A leaky damaged bottle of hand soap that was previously thrown into the <br /> dumpster by Charell Gaines was also found in the trash that had just been dumped into the compactor(IMG 8933, <br /> 8964-6). <br /> 36)Two open seemingly empty 16.9 fl oz containers of Ajax multi-purpose containers were found standing <br /> upside down in an open unlabeled blue bin, beneath a damaged cardboard box of window cleaner containers(IMG <br /> 8947). Staining was observed at the bottom of the bin beneath the Ajax containers(IMG 8945-6). <br /> 37)An open 14 fl oz container of green Ultra Ajax liquid dish soap with a broken cap was found with spillage on <br /> cardboard boxes of product(IMG 8948). Several fallen green Ultra Ajax liquid dish soap containers could be seen <br /> on the MOD grate two levels above the spillage (IMG 8949-50). <br /> 2.An open tote containing various potentially incompatible chemical containers,which were open and upside <br /> down, and appeared empty,was observed in the hazardous waste storage area(IMG 8649).A cloth was observed <br /> at the bottom of the tote, beneath the open containers.According to Sherry Aebersold, Environmental Health& <br /> Safety Compliance Manager, and Rebecca Corona, Hazmat employee,the chemical containers are opened and <br /> placed upside down in the tote to drain out any residual chemicals onto the cloth.The following chemical <br /> containers were observed inside the opened tote(IMG 8650-2): <br /> -A 16 fl oz bottle of Asssured 50% Isopropyl Alcohol <br /> -A 1 qt bottle of Diversey Crew Clinging Toilet Bowl Cleaner <br /> -A 56 fl oz bottle of LA's Totally Awesome Multi Surface Cleaner <br /> -An 8 fl oz bottle of Lysol Clean &Fresh Toilet Bowl Cleaner <br /> -A bottle of LA's Totally Awesome Carpet Spot&Stain Remover <br /> -Three 16.9 fl oz bottles of Fabuloso multi-use cleaners <br /> -A bottle of Ajax dish soap <br /> -A bottle of Palmolive dish soap <br /> -A bottle of Pine-Sol cleaner <br /> -A 1 qt, 8 oz bottle of Pine Glo cleaner <br /> FA0013602 PR0539024 SCO01 10/14/2019 <br /> EHD 22-03 Rev.9/20/2019 Page 23 of 48 RCRA Large Quantity Hazardous Waste Generator OIR <br /> 1868 E.
