<br /> Notes:
<br /> -The inspection was conducted over two days. On the first day,10/14/2019, Sherry Aebersold, Dollar Tree
<br /> Environmental Health & Safety Compliance Manager, Nikki Vangelatos, General Manager, and Julia Giddens,
<br /> Environmental Health & Safety Specialist, were present during the entire initial walk-through of the facility. Jose
<br /> Figueroa, Environmental Health & Safety Specialist,joined the walk-through in the afternoon on 10/14/2019.
<br /> Rebecca Corona, Hazmat/Hazwaste employee, was also present during parts of the walk-through on 10/14/2019.
<br /> On the second day, 10/15/2019, Sherry Aebersold was present during the entire continuation of the facility
<br /> walk-through, and provided hazardous waste paperwork for review in the afternoon. Gustavo Garcia, Operations
<br /> Manager, was present during a majority of the walk-through on 10/15/2019. Gustavo Garcia, Albert Melgoza,
<br /> Maintenance Manager, Richard Hearn, Maintenance employee, were present during the walk-through of the
<br /> Maintenance Area. Gustavo, Albert, and Robert Her, Battery Area employee, were present during the
<br /> walk-through of the Battery Area.
<br /> - Several seemingly undamaged bottles of Klar& Danver orange liquid hand soap, Assured pink liquid hand
<br /> soap, boxes of Pepsodent toothpaste were found in a STOU Hazardous Waste totes (IMG 8921-2). According to
<br /> the Dollar Tree California (CA) Store Quick Reference Guide, chemical products "become hazardous waste
<br /> when they are damaged or expired, and cannot be sold, salvaged, or used for their intended purpose."
<br /> Ensure facility employees are trained to identify sellable product from hazardous waste.
<br /> - Uniform Hazardous Waste Manifest 008628831 FILE (7/28/2016)was found on site with Ann Mollica's printed
<br /> name and signature for both the Generator's/Offeror's printed name and signature (item 15), and the Designated
<br /> Facility Owner or Operator's name and signature (item 20) (see photos IMG 9003-4). Ensure the true
<br /> generator/offeror and designated facility owner/operator are signing the manifests properly.
<br /> -The facility will be changed to from a RCRA Generator 5<25 Tons to RCRA Generator 50<250 Tons, based on
<br /> the amount of hazardous waste generated in 2018. The hazardous waste permit fee will increase.
<br /> -The inspection report was amended in the following ways: 1)Violations 118 and 119 on the inspection checklist
<br /> are combined under violation 118 on the inspection report. 2)Violations 104, 106, and 107 were added to the
<br /> inspection report.
<br /> An inspection checklist was provided to the facility operator on the day of inspection. The EHD has amended
<br /> and written the complete report on 10/28/2019, which replaces the initial checklist.
<br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement
<br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting
<br /> paperwork, by 11/27/2019.
<br /> Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate ($152). To schedule an

<br /> Provided: Training Record template (Unidocs form UN-074), List of Free CUPA Training Classes, Hazardous
<br /> Waste Labeling Minimum Requirements guidance sheet, and Return to Compliance Certification and Worksheet.
