Dollar Tree Distribution Center 1122 Runway Dr., Stockton, CA Record ID PR0539024
10/14-15//2019 CUPA—Haz. Waste
PHOTO#35
Uniform Hazardous Waste Manifest(UHMW) 012496412FLE (4/1/2019) had the waste description 11UN3139,Waste Oxidizing liquid, n.o.s. (Hydrogen
Peroxide, Phosphoric Acid), 5.1, PG Il, ERG #140)", in item 9b line 1, crossed out (IMG 9001). The number and type of containers, total quantity, unit
weight/volume, and waste codes 141 and D001 were also crossed out in line 1. This crossed out waste appeared to be replaced with the waste description
"UN1791 Hypochlorite Solutions, 9, Pg ll" and waste codes 112 and D002, in item 9b line 4, since the number and type of containers, total quantity, and
unit weight/volume remained the same. Sherry Aebersold, Environmental Health and Safety Compliance Manager, did not know why the waste description
and waste codes were changed for the waste in item 9b line 1.
Y rr••33 ■ l■ I I'
  _ M I i sx °s . f + ■IIF
• o i s
rj x `BISFj
G 1
jr
iL
d
FJ I r
I
1 'R 'F i I I •1
4 t
d=3
LR
Elianna Florido, REHS Page 35 of 130
