The following is an itemized list of underground storage tank violations that have not <br />been addressed for Harney Lane 76 as of December 06, 2021. <br />Open violations from August 18, 2021 inspection <br />Violation #102 - Failure to submit a complete and accurate application for a permit to operate a UST. <br />OBSERVATION: UST Tank Information forms are not current in the California Environmental Reporting System <br />(CERS). The primary and secondary vapor piping for diesel was reported as fiberglass. Diesel does not have vapor <br />lines. <br />REGULATION GUIDANCE: Any change of information must be updated in CERS within 30 days of the changes. <br />CORRECTIVE ACTION: Immediately log into CERS, update the required information, and submit for review by the <br />EHD. <br />❑ This violation was corrected ❑ This violation will be corrected by (date): <br />❑ Supporting documents included <br />Describe actions taken or will be taken to correct violation: <br />Violation #107 - Failure to submit and maintain complete and current financial responsibility documents. <br />OBSERVATION: Financial responsibility documents have not been submitted to the EHD. The most recent <br />financial responsibility documents in CERS are dated 7/9/2020. <br />REGULATION GUIDANCE: Current financial responsibility documents are required to be submitted annually. <br />CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, upload the required documents, and maintain a copy on site. <br />❑ This violation was corrected ❑ This violation will be corrected by (date): <br />❑ Supporting documents included <br />Describe actions taken or will be taken to correct violation: <br />Violation #112 - Failure to have properly qualified service technician test leak detection equipment every 12 <br />months. <br />OBSERVATION: Annual monitoring system certification and leak detector testing were previously performed on <br />7/7/2020 and was completed on 8/18/2021, 1 month past due. <br />REGULATION GUIDANCE: These tests are required once every 12 months. <br />CORRECTIVE ACTION: Take all necessary precautions to ensure testing is performed in a timely manner. <br />Page 1 of 7 <br />
