The facility provided two shipping records for Universal Waste(Dental Amalgam)containing Mercury and <br /> Aqueous Solution, dated 8-2018 and 4-2019. The shipping records do not indicate that the hauling company is a <br /> universal waste handler or transporter. <br /> The facility collects waste electronic devices and batteries and takes them to a recycling facility but the recycling <br /> facility was not identified. <br /> REGULATION GUIDANCE: <br /> Title 22 California Code of Regulations section 66273.39. Tracking Universal Waste Shipments <br /> Title 22 California Code of Regulations section 66273.8 Exemptions <br /> CORRECTIVE ACTION: <br /> Provide documentation that the dental amalgam is relinquished to a universal waste handler or transporter. <br /> Determine which recycling facility that electronic devices and batteries are taken to and provide that information to <br /> the EHD. Also provide the amount of electronic devices and batteries that are recycled yearly. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> An inspection checklist was provided to the facility operator on the day of inspection. A complete report was <br /> finished on February 10, 2021 and replaces the initial checklist. A revised report was completed April 14, 2021 <br /> and replaces the previous reports. <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, within 30 days of receiving the complete inspection report. <br /> An EPA ID number will be needed if the facility generates hazardous waste other than universal waste-dental <br /> amalgam and photo fixer being recycled for silver content.
