The following is an itemized list of underground storage tank violations that have not <br /> been addressed for San Joaquin General Hospital as of October 06, 2021. <br /> Open violations from August 12, 2021 inspection <br /> Violation#316-Failure to install a line leak detector. <br /> OBSERVATION: The pressurized piping on the diesel tank is not equipped with an automatic line leak detector. <br /> REGULATION GUIDANCE:Automatic line leak detectors are required to be equipped on underground pressurized <br /> piping systems that carry a hazardous substance. <br /> CORRECTIVE ACTION: Correct immediately by having a properly licensed,trained, and certified contractor install a <br /> leak detector(LG 113-x listed, if applicable)and obtain a permit within one business day from the EHD. If the leak <br /> detector can't be installed immediately,there is a possibility that the diesel UST system may be red tagged to <br /> prevent fuel inputs. <br /> ❑ This violation was corrected ❑This violation will be corrected by(date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation#319-Failure to comply with overfill prevention equipment requirements. <br /> OBSERVATION: The mechanical flapper used as the overfill prevention system is not currently functional on the <br /> diesel UST. <br /> REGULATION GUIDANCE:All UST systems must be equipped with an approved,functional overfill prevention <br /> device at all times during delivery operations. <br /> CORRECTIVE ACTION: Immediately discontinue deposition of petroleum into this tank until the component is <br /> repaired or replaced by a properly licensed, trained, and certified contractor under permit and inspection of the EHD. <br /> If the failed component can't be replaced immediately,there is a possibility that the diesel UST system may be red <br /> tagged to prevent fuel inputs. <br /> ❑ This violation was corrected ❑This violation will be corrected by(date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Page 1 of 1 <br />
