OBSERVATION: The designated operator failed to check and note all required items on the visual inspection reports.The incorrect and missing information included the following from the inspection reports: -Several questions were left blank in section VIII. -The designated operator signature was left blank. -No UDCs were listed. -Line and tank tightness testing questions were left blank. -Incorrect overfill test dates were listed. -The reports state that sump sensors were properly located, but no sumps were listed as having been inspected. REGULATION GUIDANCE: During the visual inspection,the designated operator shall check that all required testing and maintenance for the UST system have been completed and shall check all required items on the report. CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including all of the required information on the reports. This is a repeat violation, Class II. 209 CCR 2715(c)(3), 2715(c)(4) Failure to maintain a copy of the"Facility Employee Training Certificate"on-site. OBSERVATION: The Facility Employee Training Certificates list incorrect initial training dates and list"hire dates" under the"date of assuming responsibility"column. The"initial training dates"are the same as the"date of training" and do not match with previous year's records for the same employee. REGULATION GUIDANCE:All site employees must be trained prior to taking responsibility as a facility employee if after October 1, 2018, or within 30 days of hire if prior to October 1, 2018. The Facility Employee Training Certificate shall contain accurate information for all employees trained. CORRECTIVE ACTION: Update the Facility Employee Training Certificate to contain the accurate information and submit a copy of the updated form to the EHD. This is a repeat violation, Class II. 301 HSC 25292.1(a)Failure to operate UST system to prevent unauthorized releases including leaks, spills and overfills. OBSERVATION: The fuel filter under the north side of dispenser 15/16 is not located to allow drips and leaks to drain back into the UDC. A deflector plate is present on the south side, but not the north. REGULATION GUIDANCE: The underground storage tank system shall be operated to prevent unauthorized releases. CORRECTIVE ACTION: Immediately provide approved secondary containment for the filters under permit and inspection by the EHD. This is a Class II violation.
