The <br /> Hazardous Materials Inventory form for CO2 was not completed with all the required elements: Federal Hazard <br /> Codes are not selected. The Obsolete Federal Hazard Class categories were reported/identified. <br /> REGULATION GUIDANCE :This information must be current and submitted as part of the business plan. <br /> CORRECTIVE ACTIONS: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br /> This is a minor violation. <br /> 6 HSC 25505(a)(2) Failed to electronically submit a site map with all required content. <br /> OBSERVATION:A site map was not completed and submitted as part of the business plan. The site map is <br /> missing the following required elements: Loading Area, Internal Road, storm and sewer drains, access and exit <br /> points for structures and gates, evacauation staging areas, emergency response equipment, emergency shut offs, <br /> and hazardous materials handling/storage areas. <br /> REGULATION GUIDANCE:A site map shall contain a north orientation, loading areas, internal roads, adjacent <br /> streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous <br /> material handling and storage areas, and emergency response equipment. <br /> If a site map element is not applicable for your facility then list it on the map and label as"NA". <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> This is a minor violation. <br /> 9 HSC 25508.2 Failed to annually review and certify that the HMBP is complete and accurate by the due date. <br /> OBSERVATION: The business plan information has not been reviewed and resubmitted in the California <br /> Environmental Reporting System (CERS)annually by the due date, January 15. The facility did not submit their <br /> Hazardous Materials Business Plan (HMBP)for the calendar year 2020.The submittal for calendar year 2019 was <br /> submitted late on 6/14/2019.The HMBP submittal for calendar year 2021 was submitted late on 2/5/2021. <br /> REGULATION GUIDANCE: The hazardous materials inventory shall be submitted by January 15 of each calendar <br /> year and may be submitted beginning November 1 of the previous year. <br /> CORRECTIVE ACTION: Immediately log into the CERS at http://cers.calepa.ca.gov/, enter the correct or updated <br /> information, and submit to the EHD for approval. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> Conducted in-office Hazardous Materials Business Plans (HMBP) inspection.
