During your visit, you brought a <br /> few items to our attention. We have noted each issue below with our plan to address them. <br /> A) Alleged Violations: <br /> #2: Failed to initially complete and electronically submit a HMBP <br /> #3: Failed to establish and implement a hazardous materials business plan <br /> #4: Failed to electronically submit the Business Activities or Owner/Operator <br /> Identification page <br /> #5: Failed to electronically submit hazardous materials inventory for all reportable <br /> materials on site <br /> #6: Failed to electronically submit a site map with all required content <br /> #7: Failed to electronically submit an adequate ER procedures for a release or threatened <br /> release <br /> #8: Failed to establish and electronically submit a reasonable or an adequate training <br /> program <br /> #9: Failed to annually review and certify that the HMBP is complete and accurate by the <br /> due date <br /> Response: Violations 2 through 9 mentioned above have been satisfied with the completed <br /> HMBP report. This was completed and submitted into CERS on 3/10/2021 by Eric Ensminger, <br /> Verisk3E Director, Regulatory Services. Attached, please find the supporting documentation: <br /> Stockton HMBP and Stockton Completed Action Report. <br /> B) Alleged Violation: <br /> #11: Failed to provide or document initial and/or refresher training to appropriate <br /> personnel. <br /> Response: Please find the attached documentation for trainings for appropriate personnel for <br /> 2020 as well as 2021. These Documents are titled: Stockton Training Records 2.17.20 - <br /> 3.23.20, Stockton Training Records 3.18.20 - 12.31.20, Stockton Training Records 2021 YTD. <br /> NOTE: we are still in the process of completing 2021 trainings for our associates with the <br /> expectation of having them current by 3/31/2021. This information is also reflected in the <br /> Stockton Completed Action Report. <br /> C) Alleaed Violation: <br /> #12: Failed to have the HMBP readily available to facility or EHD personnel. <br /> Response: This violation has been corrected. See attachments: Stockton HMBP and Stockton <br /> Completed Corrective Action Report. <br />
