For the five-years accidental history,you stated none; please state that the facility has not had an incident that <br /> resulted in deaths, injuries, or significant property damage on site, or known offsite deaths, injuries, <br /> evacuations, sheltering in place, property damage, or environmental damage since the last RMP submittal on <br /> 1/18/2016. <br /> 2. Since you conducted the Hazard Review recently, you can update the date of the Hazard Review and the date <br /> expected to complete the recommendations( one year from the hazard review date unless approved by the <br /> CU PA). <br /> 3.
