SAN '�d DAQ II II Environmental Health Department <br /> C01-1 NTY <br /> Hazardous Materials Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> UNLIMITED COMPANY INC DBA: SMIT FARMS 12253 N CLEMENTS RD, LINDEN August 10, 2021 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 5 HSC 25505(a)(1) Failed to electronically submit hazardous materials inventory for all reportable materials on site. <br /> OBSERVATION:A 500-gallon gasoline tank used for fueling equipment, a 350-gallon diesel tank used for fueling <br /> equipment, a 500-gallon diesel tank to be used for operating a well pump, a 55-gallon drum of methanol, at least 90 <br /> gallons of propane, and an estimated 275 gallon caged tote of unknown agricultural chemical were found on site <br /> that have not been reported to CERS.Also found on site was a 500-gallon empty tank to be used for fueling <br /> equipment, according to James(JD)Salisbury, Manager. <br /> REGULATION GUIDANCE:Any material that meets or exceeds the reportable quantity shall be reported in the <br /> facility's business plan. <br /> CORRECTIVE ACTIONS: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, add the materials to the hazardous material inventory, and submit to the EHD for <br /> approval. <br /> This is a minor violation. <br /> 6 HSC 25505(a)(2) Failed to electronically submit a site map with all required content. <br /> OBSERVATION:A site map was not completed and submitted as part of the business plan. <br /> REGULATION GUIDANCE:A site map shall contain a north orientation, loading areas, internal roads, adjacent <br /> streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous <br /> material handling and storage areas, and emergency response equipment. <br /> If a site map element is not applicable for your facility then list it on the map and label as"NA". <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> This is a minor violation. <br /> FA0026394 PR0546541 SCO01 08/10/2021 <br /> EHD 19-01 Rev.9/16/2020 Page 3 of 5 Hazardous Materials Program OIR <br /> 1868 E.
