The following is an itemized list of hazardous materials violations that have not been <br /> addressed for STOCKTON DIAGNOSTIC IMAGING as of March 29 , 2021 . <br /> Open violations from February 25 , 2021 inspection <br /> ❑ This violation was corrected ❑ This violation will be corrected by ( date ) : <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation : <br /> Violation #5 - Failed to electronically submit hazardous materials inventory for all reportable materials on <br /> site . <br /> OBSERVATION MRI equipment with a maximum capacity of 10 , 940 liters of helium (storage was 87 % of capacity <br /> time of inspection ) was found on site that has not been reported to CERS for 2018 , 2019 , 2020 , and 2021 . <br /> REGULATION GUIDANCE: Any material that meets or exceeds the reportable quantity shall be reported in the <br /> facility' s business plan . <br /> CORRECTIVE ACTIONS : Immediately log into the California Environmental Reporting System ( CERS ) at <br /> http : //cers, calepa . ca . gov/ , add the materials to the hazardous material inventory , and submit to the EHD for <br /> approval . . <br /> REGULATION GUIDANCE: Any material that meets or exceeds the reportable quantity shall be reported in the <br /> facility' s business plan . <br /> ❑ This violation was corrected ❑ This violation will be corrected by ( date ) : <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation : <br /> Violation #6 - Failed to electronically submit a site map with all required content. <br /> OBSERVATION A site map was not completed and submitted as part of the business plan for 2018 , 2019 , 2020 , <br /> and 2021 . <br /> REGULATION GUIDANCE: Asite map shall contain a north orientation , loading areas , internal roads , adjacent <br /> streets , storm and sewer drains , access and exit points , emergency shutoffs , evacuation staging areas , hazardous <br /> material handling and storage areas , and emergency response equipment. <br /> If a site map element is not applicable for your facility then list it on the map and label as " NA" . <br /> CORRECTIVE ACTION : Immediately log into the California Environmental Reporting System ( CERS ) at <br /> http : //cers . calepa . ca . gov/, upload the correct or updated information , and submit to the EHD for approval . <br /> Page 2 of 5 <br />
