The following is an itemized list of underground storage tank violations that have not <br />been addressed for STOP N SHOP as of October 06, 2021. <br />Open violations from July 09, 2021 inspection <br />Violation #205 - Failure to have current UST Monitoring Plan readily accessible at the facility. <br />OBSERVATION: An approved copy of the monitoring plans were not available for inspection. <br />REGULATION GUIDANCE: A copy of these plans shall be accessible on site at all times. <br />CORRECTIVE ACTION: Immediately locate and retain a copy on site. <br />❑ This violation was corrected ❑ This violation will be corrected by (date): <br />❑ Supporting documents included <br />Describe actions taken or will be taken to correct violation: <br />Violation #206 - Failure to have a current UST Response Plan available on site. <br />OBSERVATION: An approved copy of the response plan was not available for inspection. <br />REGULATION GUIDANCE: A copy of this plan shall be accessible on site at all times. <br />CORRECTIVE ACTION: Immediately locate and retain a copy on site. <br />❑ This violation was corrected ❑ This violation will be corrected by (date): <br />❑ Supporting documents included <br />Describe actions taken or will be taken to correct violation: <br />Violation #208 - Failure to comply with the designated operator inspection requirements. <br />OBSERVATION: The designated operator failed to check and note the correct test dates for the secondary <br />containment testing on the 6/4/2021 designated operator inspection report. The incorrect date of 1/1/2018 was <br />documented. <br />REGULATION GUIDANCE: During the inspection, the designated operator shall check that all required testing and <br />maintenance for the UST system have been completed and document the dates they were done. <br />CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of <br />the required information on the reports. <br />Page 5 of 6 <br />
